Our clients frequently require access to certain common forms and exemplars. This page is designed to make such materials readily available to our clients. It is advisable that users consult with Jack Platt or another lawyer to determine the best format for any such agreement, instrument or document, and we caution anyone wishing to use such materials without sound legal advice that to do so may be improvident, imprudent or foolish. The attachments include the form of Power of Attorney that went into effect on September 12, 2010, together with the related Major Gifts Rider, instructions for preparation, and various modifications suggested by the New York State Bar Association.
